Hi,
I have Kamikaze 7.09 and no matter what I change the country to I can not access above channel 11.
I am in the uk, so have tried all of the following:
option country gb
then
option country gbr
then
option country uk
I have also changed /etc/modules.d/50-madwifi and added:
ath_ahb countrycode = 826
then
ath_ahb countrycode = gb
then
ath_ahb countrycode = gbr
Nothing works! Am I missing something?!?

maybe it is only offering the minimal channels, because there is no GB profile? .. I am also looking for the complete supported country list and there individual preset of frequencies and activated DFS/TPC ...
You may try germany for example (276). This profile includes these channels.
One reason may that your radio ist not a european one, some radios marked as "world" or "FCC comp." will only support channels up to 11.
